Cocaine users have 45 percent increased risk of glaucoma

A study of the 5.3 million men and women seen in Department of Veterans Affairs outpatient clinics in a one-year period found that use of cocaine is predictive of open-angle glaucoma, the most common type of glaucoma.

The study revealed that after adjustments for race and age, current and former cocaine users had a 45 percent increased risk of glaucoma. Men with open-angle glaucoma also had significant exposures to amphetamines and marijuana, although less than cocaine.

Patients with open-angle glaucoma and history of exposure to illegal drugs were nearly 20 years younger than glaucoma patients without a drug exposure history (54 years old versus 73 years old).

Study results appear in the September issue of Journal of Glaucoma.

"The association of illegal drug use with open-angle glaucoma requires further study, but if the relationship is confirmed, this understanding could lead to new strategies to prevent vision loss," said study first author Regenstrief Institute investigator Dustin French, Ph.D., a research scientist with the Center of Excellence on Implementing Evidence-Based Practice, Department of Veterans Affairs, Veterans Health Administration, Health Services Research and Development Service in Indianapolis. A health economist who studies health outcomes, he is also an assistant professor of medicine at the Indiana University School of Medicine.

Glaucoma is the second most common cause of blindness in the United States. Although the mechanism of vision loss in glaucoma is not fully understood, most research has focused on an increase in eye pressure gradually injuring the optic nerve. Most individuals who develop open-angle glaucoma have no symptoms until late in the disease process when substantial peripheral vision has been lost.

Dr. French and colleagues found that among the 5.3 million veterans (91 percent of whom were male) who used VA outpatient clinics in fiscal year 2009, nearly 83,000 (about 1.5 percent) had glaucoma. During the same fiscal year, nearly 178,000 (about 3.3 percent) of all those seen in the outpatient clinics had a diagnosis of cocaine abuse or dependency.

Although this study determined significant increased risk for glaucoma in those with a history of drug use, it does not prove a causal relationship. It is unlikely that glaucoma preceded the use of illegal drugs, since substance use typically begins in the teens or twenties.

"The Veterans Health Administration substance use disorder treatment program is the largest and most comprehensive program of its kind in the country," said Dr. French. He believes that the reliability of the data used in the glaucoma study reflects the overall scope and high quality of the VHA substance use program.

The long-term effects of cocaine use on intraocular pressure, the only modifiable risk factor for glaucoma, requires further study. Should the association of cocaine use and glaucoma be confirmed in other studies, substance abuse would present another modifiable risk factor for this blinding disease.

Scanner spies document secrets

ScienceDaily (Sep. 26, 2011) ? A scanner which combines the convenience of a desktop scanner with the functionality of a powerful laboratory imaging device has been developed at the University of Oxford's Faculty of Classics, and is now being commercialised by a new company Oxford Multi Spectral Limited which was recently spun out by the University's technology transfer company Isis Innovation.

The scanner was developed for imaging ancient papyri and the technology has been used to successfully scan, restore and archive over a quarter of a million historically significant manuscripts.

Oxford Multi Spectral Limited (OMS) will focus on the applications in restoring manuscripts and art, as well as the huge potential market for detecting forged security and border control documents, bank notes and forensic evidence.

Managing director of Forensic Document Services, the biggest forensic document company in the Asia Pacific, Paul Westwood, explained the Oxford scanner could be used to analyse a huge variety of samples, including crime scene samples such as counterfeit and altered documents as well as documents bearing erased or faded entries and signatures: 'The portable nature of the scanner means that it will be a great resource when document examiners are required to undertake examinations out of the laboratory environment, such as at Court Registries or the offices of opposing lawyers.

'We anticipate that using the Oxford scanner will be like moving from using a dark room to using a modern digital camera. We can use it to detect what is currently invisible and make it visible.

'The compact design and powerful imaging and analysis will be of great benefit to document examiners worldwide.'

OMS CEO, Mike Broderick said: 'OMS delivers multispectral imaging capabilities superior to large laboratory systems in a very cost-effective apparatus.

'Current multispectral imaging kits use cameras, but they are large, expensive and need specialist operators. Our scanner uses well-proven flat-bed scanner technology and powerful image processing to scan visible and 'invisible' features which absorb and reflect light at different wavelengths such as inks, pigments, polymers or papers.'

Dr Alexander Kovalchuk, the physicist who invented the scanner explained: 'An ordinary colour image has three layers: red, green and blue; a multispectral image has many more layers, some of which are invisible to the human eye, but all of these layers contain potentially useful information. Our scanner is capable of registering an unlimited number of layers.'

Dr Dirk Obbink, University Lecturer in Papyrology and head of the research group which developed the scanner said: 'The technical leaps we made mean many ancient documents which were previously unreadable can now be scanned and read.

'We can take digital images at different wavelengths of the light band and layer them on top of each other, using software to analyse them. We can set the equipment to interrogate a feature we are interested in: the surface structure, fibres, stains, watermarks, fingerprints, or alterations. We can detect an artist or writer's signature under multiple layers of paint or the pencil sketch under a watercolour.'

OMS has secured an investment of ?250,000 from a Chinese investor Changsha Yaodong Investment Consulting Co and its UK based partner RTC Innovations to commercialise, manufacture and market the scanners globally. It received ?47,600 from the University Challenge Seed Fund last year for prototyping work.

Isis Innovation managing director Tom Hockaday said: 'OMS will be the first spin-out from the University of Oxford's Faculty of Classics and indeed from the University's Humanities Division. We are delighted to see the impact of this technology across other disciplines.'

Jets' Sanchez says broken nose 'no big deal'

NEW YORK - Mark Sanchez has no concerns about his broken nose ? even if it means a few less magazine covers for now.

The Jets quarterback and recent GQ cover boy suffered what the team called a "minor break" in his nose Sunday, but he has already been cleared to practice this week without limitations and play in New York's next game at Baltimore.

"It feels a lot better now, and we'll just worry about it after the season," Sanchez said Tuesday during his weekly radio appearance on 1050 ESPN Radio. "It's no big deal."

It was the first time Sanchez broke his nose and said it "felt weird for a little bit." He was hit in the face on a sack by Oakland's Kamerion Wimbley in the third quarter, had his nose reset on the sideline and finished by wearing a visor on his helmet, something he'll do for at least the next few games.

"We'll take it game by game," Sanchez said. "We play in the elements out here, so I don't know how well a visor holds up. I've never even thought about wearing a visor before. We'll take it game by game, but I'm not worried about it either way."

He's also not concerned about the cosmetic ramifications of the injury, even after appearing on the cover of the September issue of GQ. The magazine included several photos of the bare-chested quarterback, who was razzed by his teammates and other players around the league.

"I mean, if it's a cosmetic thing, I don't really care," Sanchez said. "I'll break my nose every game as long as we're going to win. I don't care about that. It's not a big deal."

Sanchez also stood up for his coach after Hall of Fame quarterback Joe Namath suggested during a radio interview that Rex Ryan is making his team overconfident, which helped lead to its loss Sunday at Oakland. Ryan said Monday that he wouldn't change who is because of Namath's opinion, and added that the former Jets star "doesn't know our team. He's on the outside."

"I respect Joe Namath to the fullest," Sanchez said. "He's the most notable Jet of all time, the ultimate Jet, and I can understand why he would criticize the team like that, and I kind of see where he's coming from as an outsider's perspective. But you've got to understand that in this building, these guys prepare their tails off."

Sanchez acknowledged some mistakes he made in the Jets' 34-24 loss, including a costly interception after Jeremy Kerley had a long punt return that put New York deep in Oakland territory.

"It wasn't a matter of preparation," he said of the defeat.

And as far as Ryan's brash attitude, Sanchez added: "We wouldn't have it any other way, and that's the way it is."

Sanchez also brushed off comments former Jets linebacker Jason Taylor made to a Miami television station that he thinks Dolphins quarterback Chad Henne has more talent than his former New York teammate.

"He's one of the best teammates I've ever had," Sanchez said of Taylor, who spent last season with the Jets. "He's saying the right thing. That's his teammate and that's his quarterback."

New data on the genetics of aggressive prostate cancer, but ...

A new paper in the Proceedings of the National Academy of Sciences suggests the existence of a subset of ?hypermutated? cell lines in some forms of aggressive prostate cancer.

Kumar et al. carried out a series of sophisticated (?whole exome?) genetic sequencing studies on 23 cell lines derived from 16 different, lethal, metastatic prostate cancer tumors and three high-grade primary prostate cancer tumors. The full text of the paper is available on line for those with a detailed understanding of molecular genetics (or a strong sense of curiosity).

Basically, Kumar et al. were able to show the following:

  • Each tumor genome contained about 200 novel and non-synonymous gene variants.
  • Relatively few genes were mutated in the majority of tumor cell lines studied.
  • Most of these variants were specific to tumor lines from the cancers of specific individuals.
  • A few of the variants were commonly altered in tumors from different individuals.
  • Three of the prostate cancer genomes were ?hypermutated,? with 2,000 to 4,000 novel coding variants.
  • Mutations in the so-called Wnt pathway may be particularly important to development of castration resistance.

Our ability to carry out these types of highly sophisticated genetic analysis continues to open doors to understanding about why some forms of prostate cancer are more aggressive than others. However, it is important to understand that there is a ?chicken and the egg? effect going on in relation to these studies. We do not know how many of these genetic mutations may be a consequence of the development of prostate cancer or which ones may actually have a causative effect. It would be surprising if we were able to unravel this type of extraordinarily complex puzzle in the near future.

Syria wages cyber warfare as websites hacked (AP)

BEIRUT ? While Syrian protesters and security forces are engaged in a war of attrition on the ground, a different kind of battle is emerging online.

Pro- and anti-government activists in Syria are increasingly turning to the Internet, hacking and defacing websites in an attempt to win a public relations victory.

Shadowy online activist groups have hacked into at least 12 Syrian government websites in recent days, replacing their content with interactive maps and statements detailing atrocities by security forces against protesters.

The groups say their actions are in response to the regime's tactics.

Since early in the uprising, a group of pro-government hackers known as the Syrian Electronic Army has used the Internet to attack opposition activists and their perceived backers, flooding Facebook and other social networking sites with pro-regime messages like 'I love Bashar' or other. often threatening, statements.

On Monday, pro-Assad hackers briefly defaced Harvard University's website, replacing the home page with an image of Assad together with a message accusing the U.S. of supporting the uprising against him and threatening retaliation.

The hackers posted a message claiming "Syrian Electronic Army were here."

Harvard spokesman John Longrake said the attack appeared to be the work of "a sophisticated individual or group."

Other websites or Facebook pages reportedly targeted by the group include those of Oprah Winfrey, Newsweek magazine and Brad Pitt. Pitt's partner, Angelina Jolie, is a U.N. goodwill ambassador who visited thousands of Syrian refugees in Turkey in June.

"The Syrian Electronic Army has been trying to root out prominent activists in Syria and recent evidence suggests it has begun waging cyber-war against entities from countries that oppose the regime," said Anthony Skinner, associate director at Maplecroft, a British-based risk analysis company.

The Syrian Electronic Army claims on its Facebook page that it has no affiliation with the Assad regime and was founded by ordinary Syrians who want to defend the country against "fabrications and distortions of events in Syria."

But anti-government activists say they are certain the group was formed by Syrian intelligence agents and die-hard Assad supporters and volunteers.

Assad praised their efforts in a speech in June in which he lauded the role of young people in the effort, describing the group as a "real army in a virtual reality."

The group's actions were damaging at first, said Omar Idilbi, a spokesman for the Local Coordination Committees, a grass roots anti-government activist group. But the impact of the online attacks has been limited since counterattacks were launched by the hacker group Anonymous as well as two other loose groupings of hackers made up mostly of Syrian activists, the so-called Free Hackers Union and RevoluSec.

"It is an electronic war. It's legitimate. As long as it isn't hurting anyone, we are ready to wage it until the end," Idilbi said.

He said the difference between the tactics of anti-government hackers and the Syria Electronic Army was that the latter publishes threats against anti-government activists along with their phone numbers and addresses, putting their lives in danger.

RevoluSec and Anonymous said Monday they were behind the latest attacks targeting the websites of several Syrian government ministries and some major Syrian cities.

The activists said they replaced the websites with caricatures of Assad and messages that read: "Don't let Bashar monitor you online."

They also published interactive maps detailing casualty figures since the start of the uprising.

Skinner said Monday's hacking shows that the Syrian government has not erected sufficient defensive safeguards, despite reported training from its ally Iran on how to deal with the protest movement and mounting a sophisticated response.

The online attacks "underscore its vulnerability to curve-ball attacks," he said. "It will undoubtedly require a strong response from the regime on the PR front."

Anonymous said on its website that 12 government websites had been defaced by RevoluSec. Most have since been restored, but some were still down. The cartoons of Assad were removed.

"We hear that Syrian President Assad likes computers. Guess what? So do we," read a message Monday on the Twitter account of RevoluSec.

"Our goal is to raise public awareness of the abhorrent actions of the brutal Assad regime and the bloody war that it wages on its own people," a member of the group told The Associated Press Tuesday, speaking on condition of anonymity for fear of government retaliation.

Assad, a British-trained eye-doctor who succeeded his father as president, was once seen as someone who could herald reforms in Syria. Prior to becoming president in 2000, he headed Syria's Computer Society and pushed youth to become more computer-savvy.

Now activists seeking to oust him are using the Internet as a weapon against his rule, uploading graphic videos shot in secret of assaults on protesters and using social media websites to organize protests and relay messages.

Syria has banned journalists from reporting on the unrest, but videos posted online by activists have offered a rare and crucial glimpse into the far reaches of the country where the military has been deployed to crush protests.

Assad's regime tightly controls traditional media outlets in Syria, such as television, radio and newspapers. State-run channels often blame the unrest on a foreign-inspired conspiracy and Islamic extremists.

Insight: Last year's golden boy, CFTC boss Gensler under seige (Reuters)

WASHINGTON (Reuters) ? Nearly a year after launching ambitious reform plans, the top cop for U.S. futures markets, Gary Gensler, is beset by dissent from inside his agency and political attacks from outside it.

In the last two months, signs of friction within the U.S. Commodity Futures Trading Commission have spilled into public view: internal complaints by whistleblowers, and the surprise release last Thursday of a key position-limit rule by Reuters. The leak of confidential trading data has also caused waves.

The strains are not necessarily surprising for an agency that has been thrust from relative obscurity into the forefront of sweeping reforms, and is now undertaking an overhaul of derivatives trading without the big increase in funding and additional staff it is seeking.

Nonetheless, internal discord may be the biggest challenge yet for Gensler, the 53-year-old marathon runner who is sprinting to finish a new regulatory framework for the $600 trillion over-the-counter derivatives market.

And it is heating up as the CFTC draws closer to finalizing the most controversial and important rules that could force Wall Street firms to change the way they do business.

"What it tells you is there's a fair amount of acrimony within the commission on these issues," said one lawyer, noting the whistleblower complaints .

The discontent in some ways stems from the top. CFTC commissioners have openly disagreed on what the rules should look like, and the best way to put them into effect. Increasingly, some commissioners have spoken out about being left in the dark about what is going on at their own agency.

It now appears to be trickling down to lawyers, economists and others tasked with finishing the rules on which they have been working nonstop for nearly a year, and have at least six more months to go.

Jill Sommers, a Republican CFTC commissioner who has opposed a number of the rules, told Reuters on Friday she was frustrated by a lack of communication to agency commissioners about things going on at the CFTC.

"It's frustrating that we find things out third hand," Sommers said. "I understand there are a lot of different things being juggled and we have limited resources at the commission, so I'm sensitive to that, but I would guess there is a better way to keep us all on the same page."

One example was a Senate Republican aide who told Reuters last Friday that CFTC Inspector General A. Roy Lavik would investigate the whistleblower complaints on the position-limit rule. Sommers said she had not been told of the investigation.

Earlier this month, whistleblowers expressed concern at internal strife in the CFTC over how to craft a workable rule to crack down on speculation in oil markets. They asked the agency's inspector general to step in. [nS1E78D1KJ]

A second senior regulations lawyer, who like many others requested anonymity, added: "There's insurrection at the commission, evidently."

Gensler, a former Goldman Sachs executive who regularly puts in late nights and long weekends, also has been driving his staff toward the finish line on reforms that he largely helped craft, likely adding to the agency's tensions.

CFTC spokesman Steve Adamske declined to comment for this story.

FRONT-ROW SEAT

Gensler quickly became a go-to guy for Congress after being sworn in May 2009.

Lawmakers and staffers at the time lauded his ability to explain a complicated subject clearly and simply as they embarked on the Dodd-Frank financial reform legislation in the wake of the 2008 meltdown on Wall Street.

Gensler worked the halls of Congress and was a fixture on financial television as he pushed for a strengthening of the CFTC's regulatory powers. When the reform bill was before the Senate Agriculture Committee, Gensler had a front-row seat.

The tide has turned, however.

Republicans, who are chafing at new regulations in general, are particularly vocal in their criticisms of the CFTC. Democrats, already concerned the agency is not being tough enough on Wall Street and anxious for action to crack down on speculators they blame for driving up food and fuel prices, are worried the agency may be watering down the rules.

"He wanted all this stuff and put a tremendous burden on an agency that was probably not manned-up to handle it," said Craig Pirrong, a professor at the University of Houston.

"In that respect I think he is somewhat culpable, but the main issue is Congress just dumped a huge amount of stuff on the agency and I think it's inevitable that there's going to be a tremendous amount of tension," Pirrong said.

The regulatory gunslinger has sometimes rubbed people the wrong way, namely the Republicans at the CFTC who, for example, have pushed for a more detailed timeline for how the agency intends to finalize and then implement new rules -- with Gensler providing only a broad schedule.

To be sure, Gensler, who was a senior adviser for Hillary Clinton's presidential campaign, has told Congress he needs more resources and a few hundred more people to implement and enforce the swaps rules, which cover a market several times larger than the futures arena the CFTC traditionally oversees.

The Obama administration asked for funding in fiscal year 2012 to increase the agency's staff to 983 full-time employees, though it appears unlikely Congress will boost its budget enough to accommodate that request.

TOO FAST, OR NOT FAST ENOUGH

The regulator so far has finalized nearly a dozen complex rules, but most of the high-profile and controversial regulations remain works in progress.

The agency expects to consider end-user exemptions, position limits and real-time reporting this year, and rules for capital and margin, and swap-execution facilities in 2012.

Republicans, saying the CFTC has emphasized speed over deliberation in the rule-writing, have proposed legislation to slow it down. It is unlikely these bills will make it through the Democrat-led Senate.

In some cases, the challenges facing Gensler come from outside the agency. In August, Sen. Bernie Sanders, a staunch critic of oil speculators who has said the CFTC has failed to move fast enough on position limits, intentionally released oil-trading data that exposed the extensive positions speculators held in the run-up to record prices in 2008.

By law, the CFTC must hand over such information if a congressional committee acting within its proper authority requests it. There is nothing to prevent lawmakers from releasing it to the public.

Sommers called the data leak "very serious", and expressed concern that CFTC officials had said nothing about it.

Some industry watchers said the challenges facing the CFTC while writing swaps rules, which until now had been largely unregulated, are unsurprising given the complexity of the rules and differing opinions at the agency on how they should look.

"I would give him the benefit of the doubt," Jeff Harris, finance professor at Syracuse University and a former chief economist at the CFTC, said of Gensler. "He's thrown into a pretty difficult situation."

Fresh fighting erupts as Saleh returns to Yemen

President Ali Abdullah Saleh made a surprise return to Yemen on Friday after more than three months of medical treatment in Saudi Arabia in a move certain to further enflame battles between forces loyal to him and his opponents that have turned the capital into a war zone.

Saleh called for a cease-fire and said negotiations were the only way out of the crisis, but after nightfall powerful explosions shook the city as his forces shelled the strongholds of a rival tribe and a military unit that have joined the opposition. Salah's statement suggested he does not intend to step down immediately, and the opposition said he had come back only to bolster his forces in the week-old battles in Sanaa.

Saleh "returned to run the war and drive the country into an all-out civil war," Abdullah Obal, an opposition leader.

"The cannons are now speaking. Gunfire is doing all the talking," he said. "The opposition can't meet in this atmosphere. The military people are the masters of the situation now."

Saleh's return is a blow to already crumbling efforts by the United States and Saudi Arabia to work out a peaceful handover of power in the strategically placed but deeply divided and impoverished nation. Washington is eager for some sort of post-Saleh stability in hopes of continuing an alliance against al-Qaida militants in Yemen ? the terror network's most active branch, blamed in several plots for attacks on U.S soil.

The two countries have been pushing a deal by which Saleh would resign in return for immunity from prosecution. After Saleh's return, the White House held out hope for salvaging the accord.

"We urge President Saleh to initiate a full transfer of power and arrange for presidential elections to be held before the end of the year within the framework" of the agreement, said White House Press Secretary Jay Carney. "A political solution is the best way to avoid bloodshed."

Both the U.S. and Saudi Arabia were believed to be trying to keep Saleh from leaving Saudi Arabia. Obal blamed the two countries for not exerting enough pressure on Saleh to step down. He said the opposition was hardening its position in the face of Saleh's return, saying any accord "can no longer give guarantees against prosecution amid all this killing."

Saleh went to Saudi Arabia for treatment after he was severely burned over much of his body and wounded by shards of wood in a June 3 explosion at his presidential compound in the capital Sanaa. His departure fueled hopes that he would be forced to step down, but instead he staunchly refused to resign, frustrating protesters who have been taking to the streets nearly daily since February demanding an end to his 33-year old rule.

As time passed and Saleh recuperated, he was widely expected to stay in the kingdom ? and the timing of his return Friday was a surprise.

Worsening violence
This week, the deadlock that endured even during Saleh's absence broke down into the worst violence in months as forces loyal to the president's son attacked protesters in the streets and battled troops led by one of the regime's top rivals, Maj. Gen. Ali Mohsen al-Ahmar, a former Saleh aide who joined the opposition early in the uprising.

Around 100 people have been killed ? mostly protesters as regime troops hit their gathering with shelling or barrages of sniper fire from rooftops. Residents have been forced to hunker down in their homes or flee the city as the two sides exchanged bombardment over Sanaa from strongholds in the surrounding hills.

The fighting continued after Saleh returned at dawn Friday. Heavy clashes and thuds of mortars were heard throughout the night in Sanaa and into morning hours. Mortars hit the square in central Sanaa where protesters demanding Saleh's ouster are camped out, killing two. Other mortars hit a group of anti-Saleh tribal fighters in a neighborhood where battles have raged with Saleh loyalists, killing two tribesmen.

During the day Friday, thousands of Saleh supporters and opponents poured into the streets for parallel rallies in different parts of Sanaa during a lull in fighting for weekly Friday prayers.

At the pro-Saleh rally along Boulevard 70 in southern Sanaa, sermon leaders accused the opposition of attempting a coup and warned against civil war. Saleh's supporters carried his pictures along with those of the Saudi king in a tribute to the neighboring country where Saleh was recovering. Some chanted, "We love you, Ali."

At the opposition rally on Boulevard 60, demonstrators carried pictures of those killed in the violence as speakers urged security forces to stop killing their own people. "The people want the trial of the butcher," the crowd chanted.

Abdel-Hadi al-Azazi, a protest leader, warned that Saleh's return means "more divisions, more escalation and confrontations."

"We are on the verge of a very critical escalation," he told The Associated Press.

Call for dialogue
Saleh did not appear in public after his return, and state TV showed no new footage of him, though it said he was in good health. In a statement on the state news agency, Saleh called for a truce, saying, "The solution won't be through cannons and barrels, but through dialogue, understanding and ending the bloodshed," he said.

Sultan al-Barkani, the head of the ruling party's bloc in parliament and a Saleh backer, told Al-Jazeera that it was "totally unlikely" that the president will step down. "Saleh will not leave except through elections," he said.

The U.S. and Saudi Arabia have been trying to persuade Saleh to sign onto a deal proposed by Gulf Arab states, under which he would resign and hand power to his vice president to form a national unity government in return for immunity from any prosecution.

The mercurial Saleh has repeated promised to sign the agreement, then refused at the last minute.

The latest violence erupted after he recently delegated his vice president to restart negotiations with opponents on the deal. It was considered another stalling tactic by Saleh, and it was followed by a violent crackdown on protesters in Sanaa and other cities.

Yemen's turmoil began in February as the unrest spreading throughout the Arab world set off largely peaceful protests in this deeply unstable corner of the Arabian Peninsula. Saleh's government responded with a heavy crackdown, with hundreds killed and thousands wounded so far.

Cancer study boosts hopes for Roche's armed antibody (Reuters)

STOCKHOLM (Reuters) ? Women with an aggressive type of advanced breast cancer can live significantly longer without their disease getting worse if they are treated with an experimental "armed antibody" drug from Roche, researchers said on Sunday.

Data from a mid-stage clinical trial of the new combination drug, trastuzumab emtansine (T-DM1), in HER2-positive breast cancer found patients given T-DM1 had a 41 percent improvement in the time without their breast cancer worsening compared with those given trastuzumab, or Herceptin, plus chemotherapy.

Side effects were also significantly reduced in the T-DM1 group, a factor that was "particularly important and very clinically meaningful," said the study's lead investigator Sara Hurvitz, since far fewer T-DM1 patients opted to stop treatment during the phase II trial.

The results are a boost for the medicine, which the Swiss drugmaker Roche has been developing with ImmunoGen as a successor to its blockbuster Herceptin. Herceptin had sales of more than $5 billion in 2010.

"These provocative Phase II data illustrate that first-line treatment with T-DM1 provides a longer time for patients to live without cancer progression and with fewer side effects than standard chemotherapy plus trastuzumab," said Hurvitz, director of the breast oncology program at University of California, Los Angeles, who presented her findings at the European Multidisciplinary Cancer Congress (EMCC) in Stockholm.

In the trial, which involved 137 patients who had not previously had chemotherapy or HER2-targeted therapy, those on T-DM1 had an average 14.2 months without the disease worsening versus 9.2 months for those given Herceptin plus chemotherapy.

The proportion of women who stopped treatment due to side-effects was 7.2 percent in the T-DM1 arm compared with 28.8 percent in the standard therapy arm of the study.

"MAGIC LINK" IN ARMED ANTIBODY

T-DM1 is a new kind of so-called "armed antibody" drug that can carry a cell-killing payload into cancer cells.

It combines trastuzumab, an antibody and the active ingredient in Herceptin, with the agent DM1 -- a derivative of an extremely powerful type of chemotherapy called maytansine.

"Maytansine was a chemotherapy that was being developed in the 1980s, but it was so toxic that they shelved it," Hurvitz told reporters during an EMCC briefing. "So what they've done now is to figure out how to link it to the trastuzumab" -- so that it is not activated until it reaches the cancer cell.

"The magic is in the link," she said. "The whole thing is internalized within the cell. It's actually quite unique."

The fact the drug delivers its toxic payload directly into cells is also thought to be key to why it causes fewer side effects like hair loss and low white blood cell counts.

Trial data showed that only 4 percent of patients in the T-DM1 treatment group suffered hair loss, compared with 67 percent of those in the standard treatment group.

As well as having fewer unpleasant side effects, Roche believes its new drug also offers greater convenience, since it is one drug and eliminates the need to administer chemotherapy.

Commercially, it should help protect Roche's breast cancer franchise, since Herceptin could be exposed to so-called "biosimilar" generic competition in Europe from around 2015.

Consensus forecasts from Thomson Reuters Pharma show analysts see T-DM1 generating sales of $694 million by 2016.

It also keeps Roche in the breast cancer innovation race in the face of newer medicines that aim to rival Herceptin, like British drugmaker GlaxoSmithKline's Tykerb.

Oncology experts described Hurvitz's study as important and said the results were promising but would need to built upon by further late-stage studies.

Roche is conducting several Phase III trials of T-DM1 and Hurvitz said the results from one of these -- looking at T-DM1 versus GSK's Tykerb plus Xeloda, another type of chemotherapy also made by Roche -- are expected in the first half of 2012.

Hurvitz said that if those data are positive, Roche will then apply to U.S. drug regulators for a license for T-DM1.

Most Texas Job Growth Went to Immigrants | Ameriborn News

81% of Increase 2007-11 Went to New Foreign Workers

WASHINGTON (September 22, 2011) ? A new report by the Center for Immigration Studies examines job growth in Texas. Gov. Rick Perry has pointed to increased employment in Texas during the current economic downturn as one of his main accomplishments. Analysis of the Current Population Survey (CPS) collected by the Census Bureau shows immigrants (legal and illegal) have been the primary beneficiaries of this growth since 2007, and not native-born workers. This is true even though the native-born accounted for the vast majority of growth in the working-age population (age 16 to 65) in Texas.
The report is online at http://cis.org/immigrants-filled-most-new-jobs-in-Texas. Key findings:

  • Of jobs created in Texas since 2007, 81 percent were taken by newly arrived foreign workers (legal and illegal).
  • In terms of numbers, between the second quarter of 2007, right before the recession began, and the second quarter of 2011, total employment in Texas increased by 279,000. Of this, 225,000 jobs went to immigrants (legal and illegal) who arrived in the United States in 2007 or later.
  • Of newly arrived immigrants who took a job in Texas, 93 percent were not U.S. citizens. Thus government data shows that more than three-fourths of job growth in Texas went to newly arrived non-citizens (legal and illegal).
  • The large share of job growth that went to immigrants is surprising because the native-born accounted for 69 percent of the growth in Texas? working age population (16 to 65). Thus even though natives made up most of the growth in potential workers, most of the job growth went to immigrants.
  • The share of the working-age natives holding a job in Texas declined significantly from 71 percent in 2007 to 67 percent in 2011. This decline is very similar to the decline for natives in the United States as a whole and is an indication that the situation for native-born workers in Texas is very similar to the overall situation in the country despite the state?s job growth.
  • Of newly arrived immigrants who took jobs in Texas since 2007, we estimated that 50 percent (113,000) were illegal immigrants. Thus about 40 percent of all the job growth in Texas since 2007 went to newly arrived illegal immigrants and 40 percent went to newly arrived legal immigrants.
  • Immigrants took jobs across the educational distribution. Of newly arrived immigrants who took a job in Texas, more than one-third (97,000) had at least some college.
  • These numbers raise the question of whether it makes sense to continue the current high level of legal immigration and also whether to continue to tolerate illegal immigration.

Discussion: As Republicans go through the process of selecting their party?s nominee, job growth in Texas during the current economic downturn has been the subject of much discussion. GOP frontrunner Perry has argued that he has a proven record of job creation in his state, even during the current economic downturn. It is true that Texas is one of the only states where the number of people working has increased during the recession. What has not been acknowledged is that immigrants have been the primary beneficiaries of this job growth, not native-born Americans. About 40 percent job growth went to newly arrived illegal immigrants and another 40 percent to new legal immigrants.
Some may argue that it was the arrival of immigrants in the state that stimulated what job growth there was for natives. But, if immigration stimulates job growth for natives, the numbers in Texas would look very different. The unemployment rate and the employment rate (share holding job) of natives in Texas show a dramatic deterioration during the recession that is similar to the rest of the country. Among the native-born, Texas ranks 22nd in terms of unemployment and 29th in terms of its employment rate. Outside of Texas many of the top immigrant-receiving states have the worst economies. Unemployment in the 10-top immigrant-receiving states in 2011 averaged 8.7 percent, compared to 7.2 percent on average in the 10 states where the fewest immigrants arrived since 2007. These figures do not settle the longstanding debate over the economics of immigration. What they do show is that high immigration is not necessarily associated with positive labor market outcomes for the! native-born.
Some may still feel that less-educated immigrants who work at the bottom of the labor market do not really compete with natives. It is true that 56.8 percent of newly arrived immigrants had no more than a high school education. However, there are more than 3 million native-born workers in Texas who have no more than a high school education. Between 2007 and 2011 the number of native-born Texans with a high school degree or less not working increased by 259,000 and their unemployment rates nearly doubled. It would be very difficult to find evidence that less-educated workers were in short supply in the state.
It must also be remembered that many immigrants are more educated. In fact, 43.2 percent (97,000) of newly arrived immigrants who took a job in Texas had at least some college. Thus it would a mistake to assume that immigrants are only competing for jobs at the bottom end of the labor market.
Data Source: The Current Population Survey (CPS), also referred as the ?household survey?, is the source of data for this analysis. The CPS asks people at their place of residence if they are working. It also asks about their socio-demographic characteristics such as race, education level, age, citizenship, and year of arrival in the United States. Immigrants ,or the foreign-born, are those individuals who were not U.S. citizens at birth and include naturalized citizens, green card holders, guest workers, foreign students, and illegal immigrants. Following the example of other researchers, we use the demographic characteristics of illegal immigrants to estimate the number of illegal immigrants in Texas. In this analysis we compared the second quarter of 2007, before the recession began to the second quarter of 2011, the year for which the most recent data was available.

I don't get to spend a lot of time in front of the television, or the reality television, if you will, these days. A young kid, a wife, dinner with the radio on and a desire to breathe fresh air all play a part in this. But for the hour or so of surfing I do most nights before bed, I sure see a lot of bad TV (or?five minutes worth of what is most likely 30 minutes or an hour of bad TV.)

I don't have the discipline to just watch the good stuff I know I like, not when my eyes are already closing at the end of the day. I'd rather fall asleep to crap than ruin an episode of "Modern Family" by dozing in and out of it. So here's a quick, unscientific guide to the worst stuff on TV after five minutes.

'The X Factor'

That person is singing; they love her. I'm sure she'll be a huge star someday like that person who won that other singing show that wasn't that one dancing show that I don't watch either. I'd like to take everyone on this show?and lock them in a storage unit with a box full of beer cans, a couple pieces of taxidermy, some old newspapers and a Jet Ski that won't start. I'd rather watch professional poker on ESPN than another auction show. How do you pretend to create drama out of sweaty dudes picking through someone else's junk? (AP) ? The Palestinian leader took his people's quest for independence to the heart of world diplomacy Friday, seeking U.N. recognition of Palestine and sidestepping negotiations that have foundered for nearly two decades under the weight of inflexibility, violence and failure of will.

The bid to win recognition of a state in the West Bank, Gaza Strip and east Jerusalem ? submitted over the objections of the U.S. ? laid bare the deep sense of Palestinian exasperation after 44 years of Israeli occupation.

"The time is now for the Palestinian Spring, the time for independence," Palestinian President Mahmoud Abbas declared.

"The time has come to end the suffering and the plight of millions of Palestine refugees in the homeland and the diaspora, to end their displacement and to realize their rights."

After Abbas submitted his formal application, international mediators called on Israel and the Palestinians to return to long-stalled negotiations and reach an agreement no later than next year. The Quartet ? the U.S., European Union, U.N. and Russia ? urged both parties to draw up an agenda for peace talks within a month and produce comprehensive proposals on territory and security within three months.

U.S. Secretary of State Hillary Rodham Clinton said the proposal "represents the firm conviction of the international community that a just and lasting peace can only come through negotiations between the parties."

Similar plans have failed to produce a peace agreement, and it was unclear how the two sides could bridge their huge differences and resume talks.

The Quartet statement was radically different from what diplomats had been hoping to draft since it became clear that Abbas would not back down. U.S. and European officials had been trying to craft a statement that would outline parameters of the negotiations, including a reference to borders being based on the 1967 lines and affirm Israel's identity as a Jewish state.

Instead, the Quartet focused on proposing deadlines.

World sympathy for the Palestinian cause was evident from the thunderous applause that greeted Abbas as he mounted the dais in the General Assembly hall to deliver a speech that laid out his grievances against the Israeli occupation and why he felt compelled to take his appeal directly to the U.N.

In a scathing denunciation of Israel's settlement policy, Abbas declared negotiations with Israel "will be meaningless" as long as it continues building on lands the Palestinians claim. He went so far as to warn that his government could collapse if the construction persists.

"This policy is responsible for the continued failure of the successive international attempts to salvage the peace process," said Abbas, who has refused to negotiate until the construction stops. "This settlement policy threatens to also undermine the structure of the Palestinian National Authority and even end its existence."

He ignored any Palestinian culpability for the negotiations stalemate, deadly violence against Israel, and the internal rift that has produced dueling governments in the West Bank and Gaza, as well as Jewish links to the Holy Land. Some members of the Israeli delegation, including Foreign Minister Avigdor Lieberman, walked out of the hall as Abbas went to the podium.

Abbas declared himself willing to immediately return to the bargaining table, but with long-standing conditions: Israel must first stop building on lands the Palestinians claim and agree to negotiate borders based on lines it held before capturing the West Bank, Gaza and east Jerusalem in 1967. Israel rejects those conditions and has defied international pressure to freeze settlement construction. It has staked out bargaining positions that are extremely distant from anything the Palestinians would accept.

"We extend our hands to the Israeli government and the Israeli people for peacemaking," Abbas said. "Let us build the bridges of dialogue instead of checkpoints and walls of separation, and build cooperative relations based on parity and equity between two neighboring states ? Palestine and Israel ? instead of policies of occupation, settlement, war and eliminating the other.".

Israeli Prime Minister Benjamin Netanyahu, addressing the General Assembly shortly after Abbas, said his country was "willing to make painful compromises" in its quest for peace.

But while Palestinians "should live in a free state of their own," he said, they should be "ready for compromise" and "start taking Israel's security concerns seriously."

Netanyahu opposes negotiations based on the 1967 borders, saying a return to those frontiers would expose Israel's heartland to rocket fire from the West Bank. He argued that attacks on Israel from lands it occupied in south Lebanon and Gaza showed that territorial compromise would not resolve the conflict.

Talks for all intents and purposes broke down nearly three years ago after Israel went to war in Gaza, followed by the elections that propelled Netanyahu to power for a second time. A last round of talks was launched a year ago, with the ambitious aim of producing a framework accord for a peace deal. It ended three weeks later after an Israeli settlement construction slowdown expired.

The statehood bid would not deliver any immediate changes on the ground. Israel would remain an occupying force in the West Bank and east Jerusalem and continue to restrict access to Gaza, ruled by Hamas militants.

Even so, thousands of jubilant, flag-waving Palestinians, watching on outdoor screens across the West Bank, cheered their president as he made his historic speech. In Nablus, the crowd roared ecstatically when Abbas, known as Abu Mazen, told the General Assembly that he had submitted the request for full U.N. membership.

"We are here celebrating because Abu Mazen is making us a state. We want to have our own state, like any other country," said Reem al-Masri, a 30-year-old teacher who lost a brother and two cousins in fighting with Israel during the second Palestinian uprising a decade ago.

Abbas, who has never enjoyed the popular adulation accorded his predecessor, Yasser Arafat, has seen his popularity soar, allowing him to gain ground against his Hamas rivals.

U.N. chief Ban Ki-moon referred the Palestinian request to the Security Council. The U.S. and Israel have been pressuring council members to either vote against the plan or abstain. The support of nine of the council's 15 members is needed to pass, but even if the Palestinians line up that backing, the U.S. has promised to veto.

The Security Council will meet on Monday to take up the matter.

The Palestinians have said that in the absence of a positive outcome in the council, they will turn to the General Assembly, which would be expected to approve a status upgrade from permanent observer to nonmember observer state.

While more modest, this option would be valuable to the Palestinians because of the implicit recognition that negotiations would be based on lines Israel held before capturing the West Bank, east Jerusalem and Gaza in 1967. It would also give the Palestinians access to international judicial bodies such as the International Court of Justice and the International Criminal Court, where Israel fears it would be targeted unfairly.

The threat of renewed violence persisted in spite of Abbas' vow ? perceived by Israeli security officials as genuine ? to prevent it. A 35-year-old Palestinian was killed Friday in gunfire that erupted after Jewish settlers destroyed trees in a Palestinian grove and Israeli soldiers moved in.

It was not clear how serious Abbas was about his threat to dissolve the Palestinian Authority, born of the landmark accords Israel and the Palestinians signed in the 1990s. Dissolution would put 150,000 Palestinians out of work and cause chaos. Israel, which is skeptical of such talk, would be saddled with the welfare and policing of 2.5 million Palestinian subjects.
